Request Access to the Portal

A feature of the UHC's electronic medical record (EMR) is the UHC Patient Portal. A patient portal is a website that allows for secure communication between the patient and the medical practice for common interactions and transactions. The UHC Patient Portal is free-of-charge, confidential and accessible round-the-clock.

Once you have been a patient or have made an appointment at the UHC, you will be invited via e-mail to access the portal. After you have access, you will be able to:

  • access and update demographics and contact information
  • view past and future appointments
  • contact nursing staff to ask a question
  • complete appropriate forms before arriving for an office visit
  • view current medications and request prescription refills
  • request current electronic health record
  • view clinical messages
