- Complete the attestation form
- Select “View Allocation History”
- Print the allocation screen
As a full-time UNL employee, you may be eligible for exemption from the measles immunity requirement. Mark the appropriate box on the Pre-Enrollment Health Requirement Form and return it with a copy of your Allocation Confirmation printout.
