We have implemented a secure, online web portal called One Chart | PATIENT that allows you to access parts of your medical records online.
One Chart | Patient is a secure, online web portal that offers personalized access to parts of your medical record. While signing up is not required, we think you will appreciate the benefits and convenience of our online patient portal.
There is no cost.
You will be provided an activation code at clinic appointments, or you can request one from our website, www.onechartpatient.com. You will be given a one time activation code and then prompted to create a login Username and Password. As a One Chart | Patient user, you are responsible for maintaining security of your Username and Password from unauthorized individuals.

- Request/Cancel appointments
- Pay bills
- Ask your health care team questions
- Access a range of your health care information
- Most lab test results are available two business days after completed
- Most procedures results are available 30 days after completed
- Request updates to your list of medications, health problems, or allergies
- Request prescription renewals
Yes, you can request proxy access to view the One Chart | Patient account of your children and loved ones, by completing the appropriate form. The forms are located in the FAQ section of our website, www.onechartpatient.com. Once completed, please mail to the address listed on the form, and you will be contacted when the proxy access is approved.
You will be able to see much of the same information that your provider views. Some sensitive results are released at the discretion of your provider. If you do not see your results in One Chart | Patient, please call or message the ordering provider's office.

One Chart | Patient offers a messaging service with your health care team. You are able to communicate with providers that you have had an appointment with in the past two years or in the upcoming six months. We ask that you do not use One Chart | Patient to communicate serious medical concerns with your health care team. Please schedule an appointment with your provider to discuss immediate medical concerns or problems. If you are experiencing a medical emergency, call 911 immediately.
Yes. Your privacy is important to us. This website is designed to meet federal laws involving the privacy and security of your personal information. All information sent is encrypted to ensure your privacy is maintained. Additionally, the information shared is protected by layers of security within our infrastructure to further safeguard your information.
Yes. Seach and download MyChart in the Apple Store (Apple) or Market (Android). Once downloaded, open MyChart and select Nebraska Medicine as your provider. You must set up a One Chart | Patient account before using the application.
