Pre-Enrollment Health Requirement

Measles Immunization
UNL requires proof of immunity to measles of new or re-entering students prior to enrollment. The dates of two (2) Measles Mumps Rubeola (MMR) vaccinations or a blood test showing antibodies against measles is the needed proof. Attach a photocopy of your previous immunizations (i.e. records provided by your school, healthcare provider, or public health department). Please note that all immunization dates and tests must include the month, day, and year. Graduate Students
If you were previously enrolled at UNL, you do not need to complete the form unless contacted by the UHC.

Full-Time UNL Employees
You may be eligible for exemption from the measles immunity requirement. Please mark the appropriate box on the UNL Pre-Enrollment Health Requirement and return it (person, mail, fax) with a copy of your Allocation Confirmation printout (for assistance with this, call Human Resources at (402) 472-3101 to the UHC by one of the ways listed above.

International Students Only

Health Requirement Instructions

Complete this form and return it with your immunization records one of the following ways:
  • Bring it to New Student Enrollment (NSE)
  • Bring it to room 209 in the UHC
  • Fax it to (402) 472-8010

  • If you are a transfer student going through NSE for Spring 2010 enrollment, you need to have the health requirement by the following date to avoid having a hold placed on your record:

    October NSE: Nov, 30 2009
    November NSE: Dec, 14 2009
    December NSE: Jan, 8 2010
    Late Registration: Jan 18, 2010
