Welcome to the University of Nebraska-Lincoln! The University Health Center (UHC) staff would like to make your transition to college life in the United States as easy as possible. Please download the Pre-Enrollment Health Requirement Form here. Follow the instructions on the form and return the form to the UHC.

The University of Nebraska requires compliance with the following health requirements.

  • Proof of two rubeola measles/MMR immunizations, or positive lab report
  • A copy of your school immunization record or health care provider immunization record is required to show proof of two MMRs; OR
  • A laboratory report of a positive result to a blood titer test to rubeola measles.
  • PPD (tuberculosis) skin test obtained at the University Health Center
  • Required of all international students. - NO EXCEPTIONS.
  • Schedule an appointment for the PPD (tuberculosis) skin test at (402) 472-5000.
  • Two appointments are required, one for the skin test, and a second appointment two days later to check for a positive or negative reaction.
  • A chest X-ray and physician visit is required if the test is positive.
The UHC can provide the rubeola measles/MMR immunizations, a blood titer test to show immunity to rubeola measles, and the PPD test to meet the requirements. Call (402) 472-5000 for an appointment. Payment is required at the time of service.
  • Medical Insurance
  • Required of all international students.
  • The University will automatically enroll Graduate Assistants and International Students (F1 and J1) in the Healthy Option Student Insurance Plan.
  • You may waive the insurance plan if you show documentation of a comparable medical insurance plan. Proof consists of a plan in your name and the document must be in English.
  • The waiver deadline is the 14th day of classes each semester.

To help us provide the best care for you at the University Health Center, bring a copy of these documents with you to your first visit:

  • Immunization Records (school records or health care provider records of all immunizations)
  • Prescription Medications (copies of prescriptions or medication container with the pharmacy name)
  • List of Allergies
