Scheduling Your Appointment

Call 402-472-5000 to make an appointment with a provider. You may also visit the Health Center to schedule your appointment.

You will be asked to:

  • Provide basic demographic information
  • Select an appointment date and time
  • Select a specific provider or accept the first available provider
  • Describe the reason for your visit.

A specific amount of time will be allotted for your visit based on your health care needs.

Triage Nurse Visits

UHC providers are seen by appointment only. However, a triage nurse is available on a first come, first served basis for acute problems that require immediate attention.

Appointment Cancellations

Please arrive at your scheduled check in time. If you are late for your appointment, you may be asked to reschedule to a later date.

Call 472-5000 to cancel or reschedule your clinic and nurse appointments to avoid a failed visit charge. The cancellation must be at least 2 hours prior to your appointment time. Specialty Clinic visits require a 24 hour notice of cancellation to avoid a failed visit charge.

Please bring your NCard and insurance card to each visit.

Arrive at your check-in time and bring a completed medical history form.

Specialist Appointments

Your provider may refer you to a specialist when your condition requires further treatment. For your convenience, UHC contracts with community specialists who see patients at UHC.
