Appointment Cancellations
Please arrive at your scheduled check in time. If you are late for your appointment, you may be asked to reschedule to a later date.
Call 472-5000 to cancel or reschedule your clinic and nurse appointments to avoid a failed visit charge. The cancellation must be at least 2 hours prior to your appointment time. Specialty Clinic visits require a 24 hour notice of cancellation to avoid a failed visit charge.

Check-In For Your Appointment
Please bring your NCard and insurance card to each visit. Arrive at your check-in time and bring a completed medical history form. Click on the links below to download and print the appropriate form.

Scheduling Your Appointment
- Call 402-472-5000 to make an appointment with a provider. You may also visit the Health Center to schedule your appointment.
- Provide basic demographic information
- Select an appointment date and time
- Select a specific provider or accept the first available provider
- Describe the reason for your visit.

Specialist Appointments
Your provider may refer you to a specialist when your condition requires further treatment. For your convenience, UHC contracts with community specialists who see patients at UHC. Click HERE for a list of specialists and their associated charges.
Triage Nurse Visits
UHC providers are seen by appointment only. However, a triage nurse is available on a first come, first served basis for acute problems that require immediate attention.
