Getting Your Health Records

MMR Immunization Records

If you need a copy of the MMR immunization records you submitted to complete the New Student Health Requirement, this information can be found on MyRed.

  1. Go to MyRED
  2. Click on the Admissions tab
  3. Select Undergraduate Applications
  4. Under Other Links, click on Health Requirement
  5. Click Generate Immunization Record

Medical Records

Health Information Management requires at least 72 hours (three business days) to process the written request. Please allow an additional 7 to 10 business days for mailing time. Requests for records created before 1999 may take additional time to process.

If you visited the health center after July 1, 2016 (when we transitioned to our new electronic health record system) and need a copy of your medical records, the fastest and easiest way to access this information is to sign up for the patient portal, One Chart | Patient.

Once you sign up, you’ll also be able to:

  • Schedule or cancel your next appointment
  • View and pay your bill online
  • Message your health care team

If you do not wish to have an account or do not have an active account, fill out this form and mail it to the address on the form. Please note, there will be charges for requesting your medical records. Pricing is listed on the second page of the form. Medical records can also be requested online through Nebraska Medicine

CAPS Records

Fill out this form for CAPS records between July 1, 2016 to Dec. 10, 2018. Once you complete the form, email it to: UHCHIM@nebraskamed.com. You must present a photo ID when you pick up records. We cannot release records without proper identification.

Dental Records

Fill out Dental Authorization for Disclosure of Information form for dental records.

Once you complete the form, email it to: UHCDental@nebraskamed.com. You must present a photo ID when you pick up records. We cannot release records without proper identification.
