One Chart | Patient is a secure online portal where you can:

How to sign up

You will automatically receive an invitation after you schedule your first appointment. Only students who are older than 19 or 18 with a Power of Attorney form on file at the health center will receive an invitation to sign up. Due to Nebraska's age of majority law, you will be ineligible to sign up if you are 17 or younger or are 18 without a Power of Attorney form on file.
