The University Health Center has put together this comprehensive list of health-related items. Completing this checklist will help start your journey at UNL more smoothly.
Health Requirement
The University of Nebraska-Lincoln requires all new students to complete a health requirement. This health requirement is managed by the Office of the Registrar. Questions about the health requirement should be directed to registrar@unl.edu.
Domestic students
Domestic students are required to show proof of two separate rubeola measles/MMR immunizations or a positive rubeola lab result.
To complete the health requirement, log into the Immunization Form and attach the relevant immunization records. If you are unable to submit electronically, you can mail a copy of your records to:

International students
All new international students are required to complete 1.) a tuberculosis test at Nebraska Medicine–University Health Center and 2.) show proof of two separate rubeola measles/MMR immunizations or a positive rubeola lab result. It is strongly recommended students complete this requirement shortly after arriving at the university to begin classes.
Part 1: Complete the tuberculosis test
- You need your NCard at least one day before coming for your tuberculosis test.
- You do not need to schedule an appointment for the tuberculosis test. The test can be taken anytime between 9 a.m. and 1 p.m. Monday through Friday.
- When you arrive at the health center, go to the check-in desk on level two.
Part 2: Upload your measles/MMR immunization record to MyRed
If you have your vaccination records, log into the Immunization Form and attach the relevant immunization records. If you are unable to submit electronically, you can mail a copy of your records to:

I don’t have immunization records
If you do not have your immunization records, you have two options.
- Get a rubeola blood test. This blood test will look for rubeola measles antibodies to determine if a person has received the MMR vaccine.
- Receive two rubeola measles/MMR immunizations scheduled 30 days apart.
Appointments are not required for a rubeola blood test at the health center. Drop into the health center anytime Monday through Friday between 9 a.m. and 1 p.m. and tell them you need a lab test. Appointments are required for immunizations, call 402.472.5000 to schedule.
All health requirement costs are the responsibility of the student. For students who have the University of Nebraska’s student insurance plan, the cost is paid by the student insurance. For students who have another insurance plan, their insurance provider may or may not cover the cost. Students can ask their insurance provider what they will cover.
Power of Attorney Form
Power of Attorney form
In the state of Nebraska, students are not considered legal adults until the age of 19 years old. Students 18 and younger cannot consent to their own medical care without parental consent.
By completing the Power of Attorney (POA) form, the legal parent/guardian (the undersigned) is giving their minor (the 18-year-old, minor student) the power to make their own medical decisions at the student health center.
My student is 17. Can I submit a Power of Attorney form for her/him?
We can only accept POA forms for students who are 18. If your student is 17, wait until they turn 18 to fill out and submit this form to the University Health Center; until then, a staff member will call you for consent to treat if/when your student visits the health center.
What happens if I do not fill out a Power of Attorney form before my student seeks care at the University Health Center?
If a student under 19 seeks medical care at the University Health Center without a Power of Attorney form on file, a legal parent/guardian will be called at the time of check-in to complete additional forms over the phone. This will delay patient care in non-emergent situations.
How do I complete a Power of Attorney form for my 18-year-old student?
- Download and print the Power of Attorney form.
- Fill out parent/guardian’s name, parent/guardian’s city and state of residence, student’s name, student’s birth date, and student’s NUID number.
- Present the form to a notary to fill out the bottom portion. Do NOT sign until in the presence of a notary. The notary will fill out the bottom portion of the form and witness the parent/guardian sign.
- Submit the completed, notarized form to the University Health Center:

- If attending a New Student Enrollment, a health center representative may be available to submit the form for you.
Pharmacy transfers
Transfer your prescriptions
The University Health Center Pharmacy fills prescriptions from our providers, community providers and out-of-state providers. To transfer current prescriptions to the health center, call 402.472.7457. Have your prescription bottle handy as the pharmacy will ask questions for the information on the label.
Build a first aid kit
Our pharmacy sells over-the-counter items to help you put together a first aid kit to keep in your room and backpack. Some items you might want to include are:
- Allergy medication
- Antacids
- Bandages
- Cough drops
- Eye drops
- Pain relievers (acetaminophen, ibuprofen)
- Rubbing alcohol
- Thermometer
- Triple antibiotic ointment
Insurance
Services not covered by the University Program and Facilities Fee will have a charge. Charges can be billed to insurance or paid out-of-pocket.
Private Insurance
Nebraska Medicine participates with many insurance companies. Click here for a complete list. Before you visit, we recommend you check with your insurance company to make sure that Nebraska Medicine is in network with your plan.
Bring your insurance card (or a copy/picture of the front and back sides of the policyholder’s card) with you to every health center appointment.
Health insurance is not required to be treated at the health center. Students who choose not to submit to insurance or do not have insurance can receive a self-pay discount.

You will be asked about your family’s health history during your appointment. Knowing your family health history gives our providers the knowledge they need to help you manage potential health issues. Take time before you’re on campus to find out about this information.
